Skilled Nursing - RN/LPN
Provide personal care for clients in their own homes based on the client's care plan such as G-Tube feeding, tracheostomy care, dressing changes, Foley insertion, injections, pain management, medication administration. Flexible hours.
Requirements: Illinois RN or LPN license, CPR, First Aid, TB Clearance. Valid Illinois driver's license or transportation availability.

Occupational Therapist
Provides quality direct patient care. Conducts a thorough and precise assessment of the patient’s condition, functional abilities, homebound status, and environmental factors. Makes appropriate recommendations for necessary equipment, home modifications, and referrals to orders, the patient’s condition, accepted standards of practice, and Agency policies, procedures, and processes. Identify changes in the patient’s condition, consults with physician and/or patient Care Coordinator and implements appropriate interventions.
Develops and updates an individualized occupational therapy treatment plan to include education of patient’s physical condition. Develops and implements an occupational therapy treatment plan and discharge plan with the involvement of the patient and the patient care team.
Requirements: Current Illinois Occupational Therapy License,
Current CPR Certification,
Valid Illinois driver’s license or transportation availability.

Social Worker, LCSW
Job Description: The social Worker is a qualified member of the health care team who is responsible for: (1) Assessment of psychosocial and emotional factors related to the illness and care needs of patients in their homes; (2) recording clinical and progress notes; and (3) coordinating other patient care activities/services to provide the highest quality of patient care in the geographic service area to which he/she is assigned.

Requirements:
Master's Degree from a school of social work accredited by Council on Social Work Education.
Licensed to practice as a social worker in the state(s) in which he/she is required to practice, if the state has licensure practices.
A minimum of one year of social work experience in a health care setting required; previous home care experience preferred.
Speaks, writes, reads, and comprehends English.

Speech Language – Pathology
Job Description: Evaluate, assess and screen clients using appropriate standardized testing and clinical observational skills.
Provide speech/language therapy according to the mandated paperwork (i.e. IEP’s, IFSP etc)
Provide required documentation for services that your program and/or supervisor have outlined for you.
Maintain documentation in a professional, efficient, accountable and confidential manner.
Attend all meetings as scheduled and collaborate with other professionals, staff and families.
Maintain and update professional knowledge, skills and abilities through appropriate continuing education, in-service training or higher education
Carry out other duties as assigned
Requirements:
Degree in Speech/Language Pathology.
Illinois licensure in Speech/Language Pathology
ASHA certified preferred
TSHH certification
Excellent interpersonal, communication and organizational skills

Physical Therapy
Job Description: The Physical Therapist organizes and conducts physical therapy programs for the agency to facilitate rehabilitation. He /she directs client participation in selected tasks to restore, reinforce, and enhance performance. Conducts physical therapy program to restore function and prevent disability following disease, injury or loss of body part and helps patient reach their maximum physical performance.
Requirements:
The Physical Therapist must be a graduate of a physical therapy curriculum accredited jointly by the Council on Medical Education of the American Medical Association and the American Physical Therapy Association and certified by the American Physical Therapy Association under its requirements.
Illinois licensure in Physical Therapy required.
