Sunrise Rehab is flexible with it's patients in providing unique and timely care for all our clients. We work with patients and their families in scheduling hours and adjusting days in order to accommodate our patient's needs. Our services ranges from a minimum of 4 hours a day to 24 hours a day, seven days a week and providing services all year round. We work hard in order to keep our patients and family's happy. We customize our client's schedule according to their needs and availability. Whatever schedule a patient and or their family's have in mind, we work with you.

Our home care services are available at-home and foster independence in our clients, while giving peace of mind to important loved ones. Our fully bonded and insured State Certified Caregivers serve hundreds each year in Illinois. Home health care is available 24 hours a day, 7 days a week.

We are Licensed Home Health Care Services Agency, specifically established and designed to provide high quality, convenient, flexible and cost effective personal care and nursing services to residents of the surrounding communities. Sunrise Rehab Care at Home was created by the owner of Sunrise to ensure the highest quality of care. It is a home health care agency that builds on our years of eldercare experience.

Through our strong local affiliations with exceptional area health care systems, as well as our ongoing commitment to quality assurance, Care at Home is a home health care agency that strives to continually expand both the quality and scope of services and opportunities available to our clients.

Upon enrollment in any of our home health care services, a thorough assessment is conducted by one of our Registered Nurses in order to determine your specific care needs and special requests, from which a personalized care plan will be developed just for you. Your physician and your family will also participate in designing a plan for your home health care services to ensure all your needs are met.
